A.R.G. Incident, Accident & Near-Miss Investigation Training
The A.R.G. Incident, Accident & Near-Miss Investigation Training program is designed to help supervisors, managers, safety personnel, foremen, and other workplace personnel understand how to systematically investigate workplace incidents, identify contributing factors and underlying causes, document findings, and develop appropriate corrective and preventive actions.
The program emphasizes prevention and continuous improvement rather than assigning blame. Participants learn a structured approach for gathering information, interviewing personnel, evaluating workplace conditions, identifying contributing factors, analyzing causes, documenting findings, and developing corrective actions intended to reduce the likelihood of recurrence.
Program Objectives
Upon successful completion of this training, participants should be able to:
- Understand the purpose and importance of workplace incident investigation.
- Recognize the value of investigating injuries, illnesses, property damage events, and near misses.
- Understand immediate actions following a workplace incident.
- Recognize the importance of providing emergency assistance and controlling continuing hazards before beginning an investigation.
- Understand appropriate incident scene preservation considerations.
- Identify and collect relevant information, observations, photographs, documents, and other available evidence.
- Conduct effective fact-finding interviews.
- Distinguish facts, observations, assumptions, and conclusions.
- Identify direct, contributing, and underlying factors associated with an incident.
- Understand basic root cause analysis principles.
- Recognize the relationship between unsafe conditions, work practices, procedures, supervision, training, equipment, and organizational factors.
- Review applicable JHAs/JSAs, procedures, training records, inspections, and other relevant documentation.
- Develop appropriate corrective and preventive actions.
- Apply the hierarchy of controls when evaluating corrective actions.
- Assign and track corrective actions when applicable.
- Document investigation findings in a clear and organized manner.
- Communicate lessons learned and preventive measures to affected personnel.
- Understand the importance of follow-up to verify corrective actions have been implemented.

Investigation Process
Participants are introduced to a systematic investigation process that may include responding to the incident, controlling continuing hazards, preserving relevant information, gathering facts, interviewing involved personnel and witnesses, reviewing applicable documents, identifying contributing and underlying factors, developing corrective actions, documenting findings, communicating lessons learned, and verifying completion of corrective measures.
Near-Miss Investigation
The program emphasizes the importance of evaluating near-miss events and other warning indicators before they result in injuries, illnesses, property damage, environmental releases, or more serious workplace incidents.
Practical Training
When included within the agreed training scope, participants may complete incident investigation exercises using photographs, case studies, workplace scenarios, sample documentation, or simulated incident information.

Employer-Specific Training
When requested, A.R.G. may incorporate employer-specific incident reporting procedures, investigation forms, notification requirements, corrective action processes, JHA/JSA procedures, safety policies, documentation systems, and other applicable organizational requirements into the training.
Certificate of Training
Participants who successfully complete the applicable training requirements and required assessments may receive an A.R.G. Construction, Safety & Health Certificate of Training.
